A 69 year-old female patient presented with shortness of breath on a background of rheumatic heart disease, atrial fibrillation and warfarinisation for thromboembolic risk. A transthoracic echocardiogram showed a large pericardial effusion, and a pericardiocentesis obtained 1.5L of blood-stained fluid.Images
